Puerperal uterine inversion is a rare obstetric emergency that may cause maternal mortality. We describe a multiparous woman with Total uterine inversion after a normal vaginal delivery. A 28-years-old, gravid 3, pregnant woman was admitted to the hospital in the first stage of labor. She had a past medical history of curettage due to abnormal vaginal bleeding following her second vaginal delivery and the present pregnancy proceeded without complications. After the delivery, due to the history of placental adhesion, umbilical cord traction was avoided and after 20 min, the patient was asked to push hard. During a Valsalva maneuver, the uterus and the placenta were suddenly expelled from the vagina. The placenta was completely adherent to the decidua and the patient displayed no signs of shock. Then manual repositioning of the uterus was performed by a closed fist and a subTotal abdominal hysterectomy was performed. Pathological examination revealed placenta accreta and the placenta was found completely adherent at the fundus. uterine inversion usually occurs unexpectedly and is unpreventable in some cases. Assessment of the possible risk factors before delivery may help predict its occurrence. Therefore, in women with a positive history, special measures should be taken in the third stage of labor to manage the possibility of inversion.

Objective: Total uterine inversion is a rare obstetric emergency that may lead to hypovolemia and eventually death. Its incidence varies between different populations and is reported to be between 1 in 2000 to 1 in 50, 000 births. This article describes a case of acute Total uterine inversion post-partum and review of its management strategies. Case report: A primigravid 24 year old female 1. 5 hours post-partum was referred to the emergency department from the community health center with Total uterine inversion and hypovolemic shock. The patient had given birth vaginally with a midwife. We successfully performed manual reposition of the uterus and balloon tamponade was placed to stop the hemorrhage. Conclusion: Total uterine inversion is a rare but potentially deadly complication post-vaginal delivery. Its low incidence leads to sparse experience among health professionals in managing this obstetrical emergency. Early fluid resuscitation, manual reposition and balloon tamponadeis essential in order to obtain the best prognosis. Further studies are required to determine the most optimal conservative and surgical management for uterine inversion.

Chronic non-puerperal uterine inversion is a rare condition that usually presents with vaginal bleeding and vaginal mass. Frequently, it is caused by benign and malignant tumors. A mass in the vagina, particularly if it is a malignant tumor, can be confused with a uterine malignancy without inversion. Clinical diagnosis of chronic uterine inversion is difficult and may be diagnosed during surgery. In this report, we present a case of chronic uterine inversion diagnosed as a uterine sarcoma by punch biopsy.

Nonpuerperal inversion usually results from a tumor implanted on fundus of the uterus. Treatment depends on the associated pathology and the stage of the inversion. A rare case of nonpuerperal uterine inversion caused by a large fundal leiomyoma in a 38-yearold woman resulting in severe lower abdominal pain and profuse vaginal bleeding is reported. After incomplete myomectomy, reduction of the inversion vaginally was successful and then an abdominal hysterectomy was performed.
